Try deleting the includes/def_tree.php file altogether.

The def_tree.php file is essentially used as cache, so that the forum doesn't have to go to the DB every time it loads the forum. If there's a mistake in the file, then the forum can't read it and it returns errors.

Deleting the def_tree.php file tells Categories Hierarchy MOD to rewrite the file from the DB tables and therefore you get a fresh new def_tree.php file which has all the correct information in it.
