I'm no genius but the first is probably causing the other two. "permission denied" might be a chmod issue. Recheck?

Upload functions_hack_list.php fresh into includes?

okay, so far all I can find even remotely related to this is that the files just need to be re-uploaded and/or chmoded. On a side note ftp clients can be a pain as well. I've found that filezilla does not overwrite correctly, and I need to delete the file in question before uploading it again. smartftp seems to function fine as far as overwriting files. Hope that helps... Gonna do a little more digging on this, if anything turns up, will post back........

Getting stumped here. Congratulations, I think your the first one to encounter this <img> j/k I'm not having much luck w/this search though. It's a long shot, but maybe the file is corrupt. I would suggest downloading the package again, unzip, extract file in question, upload, chmod if needed. Gonna keep looking, let us know.......

EDITAnother stab in the dark. open phpmyadmin and see if the table phpbb_hacks_list is present and functioning
