New Accounts automatically added to ALL groups?

What was done before the problem appeared?

checked ranks, groups, permissions, PCP, forum permissions, sub.scriptions
De.scription and Message
There are 3 groups/ranks we made.. moderator, jr. admin and VIP
every new member is added to each of these groups automatically when activated.. serious security problem.
Any idea's on what may be causing this?
