Was wondering if it is difficult to add user colors and ranks to the online box.

Basically this is what I am thinking.

On our site we have 'Visiting Admin / Mod' and I have created a rank for this to give to users.
I want to have it show, when I change a persons rank, to also show the users different color like admins and mods and to add the rank in with the other ranks.

Or is there a different way to do this?

this would not be a simple task to do yourself, but the Advanced Group Color Management - IM 141 mod will allow you to do this.
