[RESOLVED] Parse error when adding YouTube mod

When I add the YouTube bbcode mod to my site, I get the following error at the top of the center portal blocks, the bottom of the forum list, and at the top of the shoutbox.

Parse error: syntax error, unexpected '<' in /home/marysvil/public_html/zz-horn/includes/bbcode.php(67) : eval()'d code on line 1

Also, everything seems to function when I try to add the YouTube link, but when the post is submitted, the area where the video should be is blank, I'm assuming due to this error.

When I replace the bbcode.tpl file from my template's directory with the original, the error goes away, but obviously, still nothing works.

The YouTube button is showing in the posting screen.

I aalso have a block on my portal page that uses bbcode to provide RSS links. It uses nothing but images and links, but it doesn't display correctly, either when this error appears.

I've removed the bbcode.tpl file and posting_body.tpl files in the meantime.

Here are the screenshots and I've attached my bbcode.php file. I tied removed my comments from the bbcode.php file to see if that made a difference, but it does not.

youtube is already included in 141, however it appears that a few files were overwritten in one of the updates. here are the correct files IM141 files
